Your First Visit at Martin Dentistry

During your first visit to Martin Dentistry, you’ll receive a thorough dental exam and X-rays as part of your comprehensive care. We’ll take the time to discuss your dental and medical history, address any concerns and collaborate with you on a personalized treatment plan to support your long-term oral health.

Our comfortable, family-friendly offices are designed to make your experience positive from the start. With a welcoming environment and state-of-the-art facilities, we’re committed to building lasting relationships. We’re here to answer all your questions and provide you with any dental patient resources you need for ongoing care.

Please bring the following to your first appointment:

• Completed new-patient forms
• Any X-rays from a previous dentist (or have them forwarded to our office)
• A valid form of ID
• A list of medications you currently take
• A list of current health concerns, issues and past surgeries
• Your dental insurance card (if applicable)

Insurance & Financing

Everyone deserves access to affordable dental care, and we’re committed to making that possible. We accept most major dental insurance plans and offer an in-house membership savings plan for patients without insurance.
